Diabetes

Diabetes is a group of conditions linked by an inability to produce enough insulin and/or to respond to insulin. This causes high blood glucose levels (hyperglycemia) and can lead to a number of acute and chronic health problems, some of them life-threatening.

People with diabetes are unable to process glucose, the body's primary energy source, effectively. Normally, after a meal, carbohydrates are broken down into glucose and other simple sugars. This causes blood glucose levels to rise and stimulates the pancreas to release insulin into the bloodstream. Insulin is a hormone produced by the beta cells in the pancreas. It regulates the transport of glucose into most of the body's cells and works with glucagon, another pancreatic hormone, to maintain blood glucose levels within a narrow range.

If someone is unable to produce enough insulin, or if the body's cells are resistant to its effects (insulin resistance), then less glucose is transported from the blood into cells. Blood glucose levels remain high but the body's cells "starve." This can cause both short-term and long-term health problems, depending on the severity of the insulin deficiency and/or resistance. Diabetics typically have to control their blood glucose levels on a daily basis and over time to avoid health problems and complications. Treatment, which may involve specialized diets, exercise and/or medications, including insulin, aims to ensure that blood glucose does not get too high or too low.

  • A very high blood glucose level (acute hyperglycemia) can be a medical emergency. The body tries to rid the blood of excess glucose by flushing it out of the system with increased urination. This process can cause dehydration and upset the body's electrolyte balance as sodium and potassium are lost in the urine. With severe insulin deficiency, glucose is not available to the cells and the body may attempt to provide an alternate energy source by metabolizing fatty acids. This less efficient process leads to a buildup of ketones and upsets the body's acid-base balance, producing a state known as ketoacidosis. Left unchecked, acute hyperglycemia can lead to severe dehydration, loss of consciousness, and even death.
  • A very low blood glucose level (hypoglycemia), often as a result of too much insulin, can also be life-threatening. It can lead to hunger, sweating, irregular and rapid heart beat, confusion, blurred vision, dizziness, fainting, and seizures. Severely low blood glucose can lead fairly quickly to insulin shock and death.
  • Glucose levels that rise over time and become chronically elevated may not be initially noticed. The body tries to control the amount of glucose in the blood by increasing insulin production and by eliminating glucose in the urine. Signs and symptoms usually begin to arise when the body is no longer able to compensate for the higher levels of blood glucose.

Chronic high blood glucose can cause long-term damage to blood vessels, nerves, and organs throughout the body and can lead to other conditions such as kidney disease, loss of vision, strokes, cardiovascular disease, and circulatory problems in the legs. Damage from hyperglycemia is cumulative and may begin before a person is aware that he or she has diabetes. The sooner that the condition is detected and treated, the better the chances are of minimizing long-term complications.

Type 1 Exact cause unknown; thought to be primarily an autoimmune disease that involves the destruction of the insulin-producing beta cells in the pancreas; can occur at any age but usually diagnosed in children and young adults.
Type 2 Most common type; associated with insulin resistance and with insulin production that is insufficient to meet the body's needs and to compensate for resistance. It develops most frequently in overweight middle-aged and elderly people. With increased obesity in children and adolescents, the condition is becoming more common at younger ages.
Gestational Develops during a woman's pregnancy and affects both mother and developing baby; typically develops late in the pregnancy
Prediabetes Higher blood glucose than normal, but not considered diabetes; people with prediabetes are at an increased risk of developing diabetes.
Others A group of less common types of diabetes. Any condition that damages the pancreas and/or affects insulin production or usage can cause diabetes.

Hypertension

Hypertension, also known as high blood pressure, is a persistent elevation in blood pressure that taxes the heart. Over time, it can cause serious damage to the heart as well as other organs, such as the kidneys, brain, and eyes.

Blood pressure (BP) is the force that blood exerts on the walls of the arteries. It depends on the strength and rate of the heart's contraction as it pumps blood and on the resistance to the flow of blood through the arteries. The amount of resistance depends on the elasticity and diameter of the blood vessels and the volume of blood flowing through them. The narrower the arteries and the more blood pumping through them, the higher the blood pressure will be. Maintaining a healthy lifestyle can help delay or prevent hypertension.

Blood pressure rises and falls during the day depending on a person's level of activity and physical and emotional stress. Largely controlled by the autonomic nervous system (the part of the nervous system that controls involuntary actions), BP is also affected by several different hormones. When one or more of these hormones is out of balance, it can cause high blood pressure or hypertension.

The hormones include:

  • Angiotensin II - this hormone causes arteries to contract, making them temporarily narrower and increasing the pressure of the blood flowing through them.
  • Aldosterone - this hormone is produced by adrenal glands (mainly stimulated by Angiotensin II) and helps regulate the amount of sodium, potassium, and fluids excreted by the kidneys.
  • Catecholamines - this group of hormones, including, for example, dopamine and epinephrine (adrenaline), are produced by the brain and adrenal glands in response to stress. They increase heart rate and resistance in blood vessels.

Measuring BP takes into account two pressures, measured in millimeters of mercury (mm Hg). The first, systolic pressure, is the force exerted on the blood vessel walls when the heart is pumping blood. Diastolic pressure reflects the force present when the heart relaxes between beats. They are written as systolic over diastolic pressure. For instance, a blood pressure of 120/80 mm Hg or 120 over 80 corresponds to a systolic pressure of 120 and a diastolic pressure of 80.

A single measurement of blood pressure is not diagnostic. Typically, multiple readings are taken on different days and, if measurements are consistently high, a diagnosis of high blood pressure is made.

Usually, diastolic pressures will mirror systolic pressures, but as people age, the diastolic pressure tends to level out. Then, the form of hypertension that involves primarily the systolic pressure (called isolated systolic hypertension) becomes more common. In general, the greater the blood pressures for extended periods of time, the greater the potential for damage.

The classifications of blood pressure based on recommendations of the National Heart Lung and Blood Institute (NHLBI) Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC 7) are summarized in the table below.

Thyroid

Thyroid diseases are primarily conditions that affect the amount of thyroid hormones being produced. Women are more likely than men to have thyroid problems, with 1 in 8 developing a thyroid disorder during her life. The following is a list of the more common thyroid disorders.

Diseases that present as abnormal thyroid function:

  • Hypothyroidism : too little thyroid hormone; slowing of body functions; symptoms include weight gain, dry skin, constipation, cold intolerance, puffy skin, hair loss, fatigue, and menstrual irregularity in women. Severe untreated hypothyroidism, called myxedema, can lead to heart failure, seizures and coma. In children, hypothyroidism can stunt growth and delay sexual development. Specific types of hypothyroidism include:
    1. Congenital hypothyroidism : this condition affects infants from birth; it is caused by inadequate thyroid hormone and is most commonly due to a thyroid gland that is missing, only partially developed, or located in an abnormal part of the body. The rest of the cases are due to a normal-sized or enlarged thyroid that does not function properly or produce sufficient thyroid hormone. Left untreated, this condition can cause delays in physical and intellectual development. Hypothyroidism testing is performed in the United States as part of newborn blood screening programs since early detection and treatment can minimize long-term damage.
    2. Hashimoto thyroiditis : Aautoimmune condition in which the immune response targets the thyroid, causing inflammation and damage and the production of autoantibodies. With Hashimoto thyroiditis, however, the thyroid produces low amounts of thyroid hormone.
    3. Iodine deficiency : lack of this element (as iodide) diminishes the ability of the thyroid gland to make enough thyroid hormone. T4 has four iodides and T3 has three. Iodide is present throughout the environment, but most iodide is in sea water and seaweed. People who live in countries with little access to the sea often have iodide deficiency unless their sources of food are supplemented with iodide. Fortunately, iodide is used to fight bacterial growth in many foods (such as iodized salt) and is also in many dietary supplements.
  • Hyperthyroidism : too much thyroid hormone; sometimes called "overactive thyroid;" acceleration of body functions; symptoms include increased heart rate, anxiety, weight loss, difficulty sleeping, tremors in the hands, weakness, and sometimes diarrhea. There may be puffiness around the eyes, dryness, irritation, and, in some cases, bulging of the eyes. The affected person may experience light sensitivity and visual disturbances.
    Because the eyes may not move normally, the person may appear to be staring. Specific types of hyperthyroidism include:
    1. Graves disease : the most common cause of hyperthyroidism; it is a chronic autoimmune disorder in which the affected person's immune system produces antibodies that act like TSH, stimulating the thyroid to produce excessive amounts of thyroid hormone.
    2. Thyroid tumor : a small benign tumor may become insensitive to the negative feedback of low TSH and continue to produce excess thyroid hormone.
    3. Abnormal thyroid stimulation: a tumor of the cells that produce TSH can result in excess thyroid hormone production. Also, human chorionic gonadotropin (hCG), the hormone that supports the growth of the fetus in pregnancy, can act like TSH and sometimes produce hyperthyroidism in pregnant women, especially if their hCG levels are very high.
  • Goiter : a visible enlargement of the thyroid gland; in the past, this condition was relatively common and was due to iodine deficiency but, with iodine supplementation of food. In other parts of the world, however, iodine-related goiters are still common and represent the most common cause of hypothyroidism in some countries. Goiters may compress vital structures of the neck, including the trachea and esophagus. This compression can make it difficult to breathe and swallow. Any of the diseases listed above can cause goiters. A rare cause is thyroid hormone resistance syndrome, in which a mutation in the thyroid hormone receptor decreases thyroid hormone function.
  • Thyroiditis : an inflammation of the thyroid gland; it may be associated with either hypo- or hyperthyroidism. It may be painful, feeling like a sore throat, or painless. Thyroiditis may be due to an autoimmune disorder (especially Hashimoto thyroiditis), an infection, exposure to a chemical that is toxic to the thyroid, or due to an unknown cause (idiopathic). Depending on the cause, it can be acute but transient or chronic.

Diseases that present as thyroid tumors:

  • Thyroid nodule : a small lump on the thyroid gland that may be solid or a fluid-filled cyst; these nodules are common and the overwhelming majority of them are harmless. Occasionally, however, thyroid nodules can be cancerous and need to be treated.
  • Thyroid cancer : thyroid cancer is fairly common and its incidence has been increasing over the past few decades. It is 8th most common type of cancer and the fastest growing type. There are four main types of thyroid cancers:
    1. Papillary thyroid cancer - about 80% of thyroid cancer cases are papillary. This type affects more women than men and is more common in younger people.
    2. Follicular thyroid cancer - about 15% of thyroid cancers are follicular, a more aggressive type of cancer that tends to occur in older women.
    3. Anaplastic thyroid cancer, also found in older women, accounts for about 2% of thyroid cancers and tends to be both aggressive and difficult to treat.
    4. Medullary thyroid cancer (MTC) - accounts for 3% of thyroid cancers and is malignant; it can spread beyond the thyroid and be difficult to treat if it is not discovered early. MTC produces excess calcitonin and may be found alone or linked with other endocrine cancers in a syndrome called multiple endocrine neoplasia syndromes. The cells that make calcitonin are different from the cells that make thyroid hormone. These may grow in number, resulting in a disorder called C-cell hyperplasia. This is a benign condition that also produces excess calcitonin; it may or may not progress to become medullary thyroid cancer.
    5. Lymphoma, a tumor composed of lymphocytes (the cells that produce immunity from bacteria and viruses), can also occur in the thyroid.

Cholesterol

It may surprise you to know that cholesterol itself isn't bad. In fact, cholesterol is just one of the many substances created and used by our bodies to keep us healthy.

What is cholesterol?
  • Cholesterol is a waxy substance that comes from two sources: your body and food. Your body, and especially your liver, makes all the cholesterol you need and circulates it through the blood. But cholesterol is also found in foods from animal sources, such as meat, poultry and full-fat dairy products. Your liver produces more cholesterol when you eat a diet high in saturated and trans fats.
  • Excess cholesterol can form plaque between layers of artery walls, making it harder for your heart to circulate blood. Plaque can break open and cause blood clots. If a clot blocks an artery that feeds the brain, it causes a stroke. If it blocks an artery that feeds the heart, it causes a heart attack. View an animation of cholesterol
    There are two types of cholesterol: "good" and "bad." Too much of one type — or not enough of another — can put you at risk for coronary heart disease, heart attack or stroke. It's important to know the levels of cholesterol in your blood so that you and your doctor can determine the best strategy to lower your risk.
  • Making healthy eating choices and increasing exercise are important first steps in improving your cholesterol. For some people, cholesterol-lowering medication may also be needed to reduce the risk for heart attack and stroke.
What Can Cholesterol Do?
  • High cholesterol is one of the major controllable risk factors for coronary heart disease, heart attack and stroke. As your blood cholesterol rises, so does your risk of coronary heart disease. If you have other risk factors such as smoking, high blood pressure or diabetes, this risk increases even further. The greater the level of each risk factor, the more that factor affects your overall risk. Your cholesterol level can be affected by your age, gender, family health history and diet.
  • When too much LDL (bad) cholesterol circulates in the blood, it can slowly build up in the inner walls of the arteries that feed the heart and brain. Together with other substances, cholesterol can form a thick, hard deposit called plaque that can narrow the arteries and make them less flexible. This condition is known as atherosclerosis. If a clot forms and blocks a narrowed artery, a heart attack or stroke can result.
High Cholesterol - Risk
  • LDL (bad) cholesterol is produced naturally by the body, but many people inherit genes from their mother, father or even grandparents that cause them to make too much. Eating foods with saturated fat or trans fats also increases the amount of LDL cholesterol in your blood. If high blood cholesterol runs in your family, lifestyle modifications may not be enough to help lower your LDL blood cholesterol.
Symptoms, Diagnosis & Monitoring
  • Many people do not know their cholesterol is too high because there are usually no symptoms. That's why it is important to have your cholesterol levels checked by your doctor.
    Talk with your healthcare provider about assessing your risk for a heart attack or stroke. Cholesterol levels are an important factor in estimating your personal risk.
  • Visit your healthcare provider to create an action plan that will help you make important lifestyle changes. Sometimes, medication is needed in addition to a healthy diet and lifestyle.
Prevention and Treatment
  • Too much cholesterol in the blood can lead to cardiovascular disease. The good news is, you can lower your cholesterol and reduce your risk of heart disease and stroke. Working with your doctor is key. It takes a team to develop and maintain a successful health program. You and your healthcare professionals each play an important role in maintaining and improving your heart health.
  • Work with your doctor to determine your risk and the best approach to manage it. In all cases, lifestyle changes are important to reduce your risk for heart attack and stroke. In some cases, cholesterol-lowering statin medicines may also provide benefit.
  • Learn how to make diet and lifestyle changes easy and lasting. Also make sure you understand instructions for taking medication because it won't work if you don't take it as directed.

Cancer

Cancer begins in your cells, which are the building blocks of your body. Normally, your body forms new cells as you need them, replacing old cells that die. Sometimes this process goes wrong. New cells grow even when you don't need them, and old cells don't die when they should. These extra cells can form a mass called a tumor. Tumors can be benign or malignant. Benign tumors aren't cancer while malignant ones are. Cells from malignant tumors can invade nearby tissues. They can also break away and spread to other parts of the body.

Cancer is not just one disease but many diseases. There are more than 100 different types of cancer. Most cancers are named for where they start. For example, lung cancer starts in the lung, and breast cancer starts in the breast. The spread of cancer from one part of the body to another is called metastasis. Symptoms and treatment depend on the cancer type and how advanced it is. Most treatment plans may include surgery, radiation and/or chemotherapy. Some may involve hormone therapy, biologic therapy, or stem cell transplantation.

What Is Stress?

Stress is your body's way of responding to any kind of demand. It can be caused by both good and bad experiences. When people feel stressed by something going on around them, their bodies react by releasing chemicals into the blood. These chemicals give people more energy and strength, which can be a good thing if their stress is caused by physical danger. But this can also be a bad thing, if their stress is in response to something emotional and there is no outlet for this extra energy and strength.

What Causes Stress?

Many different things can cause stress -- from physical (such as fear of something dangerous) to emotional (such as worry over your family or job.) Identifying what may be causing you stress is often the first step in learning how to better deal with your stress. Some of the most common sources of stress are:

Survival Stress

You may have heard the phrase "fight or flight" before. This is a common response to danger in all people and animals. When you are afraid that someone or something may physically hurt you, your body naturally responds with a burst of energy so that you will be better able to survive the dangerous situation (fight) or escape it all together (flight). This is survival stress.

Internal Stress

Have you ever caught yourself worrying about things you can do nothing about or worrying for no reason at all? This is internal stress and it is one of the most important kinds of stress to understand and manage. Internal stress is when people make themselves stressed. This often happens when we worry about things we can't control or put ourselves in situations we know will cause us stress. Some people become addicted to the kind of hurried, tense, lifestyle that results from being under stress. They even look for stressful situations and feel stress about things that aren't stressful.

Environmental Stress

This is a response to things around you that cause stress, such as noise, crowding, and pressure from work or family. Identifying these environmental stresses and learning to avoid them or deal with them will help lower your stress level.

Fatigue and Overwork

This kind of stress builds up over a long time and can take a hard toil on your body. It can be caused by working too much or too hard at your job(s), school, or home. It can also be caused by not knowing how to manage your time well or how to take time out for rest and relaxation. This can be one of the hardest kinds of stress to avoid because many people feel this is out of their control.

How Does Stress Affect You?

Stress can affect both your body and your mind. People under large amounts of stress can become tired, sick, and unable to concentrate or think clearly. Sometimes, they even suffer mental breakdowns.

Bad stress - "good" or "positive" stress?

Understanding your stress level is important. If nothing in your life causes you any stress or excitement, you may become bored or may not be living up to your potential. If everything in your life, or large portions of your life, cause you stress, you may experience health or mental problems that will make your behavior worse.

Recognizing when you are stressed and managing your stress can greatly improve your life. Some short-term stress - for example what you feel before an important job presentation, test, interview, or sporting event - may give you the extra energy you need to perform at your best. But long-term stress - for example constant worry over your job, school, or family - may actually drain your energy and your ability to perform well.

  • Millions of people suffer from stress each year.
  • In fact, 3 out of 4 people say they experience stress at least twice a month.
  • Over half of those people say they suffer from 'high' levels of stress at least twice a month.
  • Stress can contribute to heart disease, high blood pressure, strokes, Alzheimer's disease, diabetes, gastrointestinal problems and asthma. You also more likely to catch less serious illnesses like colds. It can also contribute to alcoholism, obesity, drug addiction, cigarette use, depression, and other harmful behaviors.
  • In the last 20 years, the number of people reporting that stress affects their work has gone up more than four folds. (Whereas the number of people reporting that other illnesses affect their work have gone down.)
Fight Back Against Stress - and Improve Your Health
  1. Breathe deeply. Just a few minutes of deep breathing can calm you and tame the physiologic stress response. While building in a specific time to relax each day is a good idea, one advantage to deep breathing for stress relief is that you can do it anywhere - at your desk or in your (parked) car, for instance.
    You breathe out; you relax a specific muscle group. Start with the muscles in your jaw. On the next breath out, relax your shoulders. Move through the different areas of your body until you're feeling calm.
  2. Focus on the moment. When you're stressed, you're probably living in the future or the past. You're worried about what to do next or regretful about something you've already done. To get some stress relief, instead try focusing on what you're doing right now. You can calm yourself by bringing yourself back to the present moment. If you're walking, feel the sensation of your legs moving. If you're eating, focus on the taste and the sensation of the food.
  3. Reframe the situation. So you're already running late and then find yourself stuck in terrible traffic. Getting worked up is a natural reaction, but it won't help you at all. Rather than swearing and pounding the steering wheel, get a different perspective. Look at that time as an opportunity - a few minutes to yourself where you don't have any other obligations.
  4. Keep your problems in perspective. It might seem Pollyannaish, but the next time you're feeling stressed out, think about the things for which you're grateful.
    We get stressed when we focus so much on a specific problem that we lose perspective. You need to remind yourself of the basic ways in which you're lucky - that you have family and friends, that you can see, that you can walk. It can be a surprisingly effective method for stress relief.

While these stress management techniques can help in the moment, you can also make a few larger changes to your way of life. Regular exercise is key to long-term stress management. People who exercise tend to have better moods and more energy than people who don't. What's more, regular exercise will independently lower your risks for many health problems.

Learning some relaxation techniques, meditation, or yoga will help with stress management, too. Getting good at any of these approaches will take a little time and practice, but the payoff - for your short-term mood and long-term health - could be substantial.
